Weight Loss Pen FAQs: Your Top 15 Questions Answered

If you're considering a weight loss pen or have recently started one, chances are you have questions. Here are the 15 most common questions we hear — answered clearly and honestly.

1. What is a weight loss pen?

A weight loss pen is a pre-loaded injectable device containing a GLP-1 receptor agonist medication — typically semaglutide or tirzepatide. These medications mimic a natural gut hormone that regulates appetite, slows digestion, and helps the body process blood sugar more efficiently. They are administered as a once-weekly subcutaneous (under the skin) injection and are prescribed for adults with a BMI of 30+, or 27+ with a weight-related health condition.

2. How much weight can I expect to lose?

Clinical trials have shown average weight loss of:

  • Semaglutide (Wegovy): ~15% of body weight over 68 weeks
  • Tirzepatide (Mounjaro/Zepbound): ~20% of body weight over 72 weeks

Individual results vary based on starting weight, diet, exercise, dose, and adherence. Most people lose 1–2 lbs per week on average, though progress is rarely perfectly linear.

3. How quickly does a weight loss pen start working?

Most people notice reduced appetite within the first 1–2 weeks of starting. Visible weight loss typically begins in weeks 2–4, though the most significant results build over months 3–12 as the dose increases to the full therapeutic level.

4. Do I have to inject myself?

Yes — weight loss pens are self-administered injections. The needle is very fine (much smaller than most people expect), and the injection is subcutaneous (into the layer of fat just under the skin), not into muscle or vein. Most users report that it is far less uncomfortable than anticipated, and it becomes routine within a few weeks.

5. Where do I inject?

Approved injection sites are:

  • Abdomen (at least 5cm from the navel)
  • Upper thigh
  • Upper arm (outer area)

Rotate between sites each week to prevent skin thickening.

6. What are the main side effects?

The most common side effects, particularly in the first few weeks, are:

  • Nausea
  • Constipation or diarrhoea
  • Reduced appetite (intended effect)
  • Fatigue
  • Mild indigestion or bloating

These typically reduce significantly after the first 4–8 weeks. Eating smaller meals, staying hydrated, and avoiding greasy or spicy foods helps minimise them.

Rare but serious side effects include pancreatitis and gallbladder issues — contact your doctor if you experience severe abdominal pain.

7. Do I need a prescription?

Yes. GLP-1 weight loss medications are prescription-only in the UK, US, and most countries. You will need to be assessed by a qualified healthcare provider — either a GP, specialist, or licensed telehealth provider — before being prescribed.

8. Can I use a weight loss pen if I have type 2 diabetes?

GLP-1 medications were originally developed specifically for type 2 diabetes management. Many people with type 2 diabetes use them for both blood sugar control and weight loss. If you have diabetes and are on other medications — particularly insulin or sulfonylureas — your doctor will need to adjust your regimen to avoid hypoglycaemia. Always inform your prescriber of all medications you are taking.

9. Is it safe to use long term?

GLP-1 medications have been used clinically for over 15 years (initially for diabetes). Long-term safety data is extensive for this medication class. In fact, cardiovascular outcome trials have shown that semaglutide significantly reduces the risk of major cardiac events in people with cardiovascular disease.

Current guidance supports long-term use for weight management, similar to how blood pressure or cholesterol medications are used ongoing.

10. Will I regain the weight if I stop?

For many people, yes — some weight regain occurs after stopping GLP-1 medications, because the appetite-regulating effects of the medication are no longer present. Studies have shown average regain of approximately two-thirds of lost weight within a year of stopping, in the absence of other interventions.

This is why lifestyle habits — improved diet, regular exercise, behavioural change — built during treatment are so important. They provide a foundation that reduces (though may not eliminate) regain after stopping.

11. Can I drink alcohol on a weight loss pen?

Alcohol is not strictly contraindicated with GLP-1 medications, but there are important considerations:

  • Alcohol slows digestion further (on top of the medication's effect), which can worsen nausea
  • Alcohol is calorie-dense and can undermine your calorie deficit
  • Blood sugar interactions are possible, particularly if you also have diabetes
  • Alcohol disrupts sleep, which affects hunger hormones

Modest, occasional alcohol is generally fine — but heavy or regular drinking is likely to impair your results and increase side effect risk.

12. What's the difference between semaglutide and tirzepatide?

Both are GLP-1 receptor agonists, but there is a key difference:

  • Semaglutide (Wegovy, Ozempic) activates the GLP-1 receptor only
  • Tirzepatide (Mounjaro, Zepbound) activates both the GLP-1 and GIP receptors — a "dual agonist" approach

In head-to-head comparisons, tirzepatide has generally demonstrated greater average weight loss (20%+ vs ~15% for semaglutide), though individual responses vary. Tirzepatide is newer to the weight loss market and may be priced higher.

13. Can I use a weight loss pen if I'm pregnant or breastfeeding?

No. GLP-1 medications are contraindicated during pregnancy and breastfeeding. If you become pregnant while using a weight loss pen, discontinue immediately and contact your healthcare provider. Women of childbearing age should use effective contraception during treatment.

14. How do I store my weight loss pen?

  • Unused pens: Refrigerate at 2°C–8°C (do not freeze)
  • In-use pens: Can be kept at room temperature (up to 30°C) for 4–8 weeks depending on the product
  • Travelling: Carry in hand luggage; use an insulated cool pouch for longer journeys
  • Disposal: Use a sharps container — never household waste

The process is straightforward:

  1. Consult a healthcare provider — GP, private clinic, or telehealth service
  2. Get assessed — BMI, health history, medications, contraindications
  3. Receive your prescription and first pen
  4. Start at the lowest dose and titrate upward as directed
